    By completing this form, you are agreeing to be billed monthly through FACTS.
    Charges will begin with August billing.

    • Bus Service One Way / $77.78 monthly
    • Lunch / $94.44 monthly
    • Beverage only/ $20 monthly
    • Milk Break for K3-3rd / $20 monthly
    • Art (K5-5th) / $60 monthly
    • Ballet / *prices vary by class *10 month pay
    • Extended Care (K3/K4) 11:30-3:00 / $255.56 monthly
    • Extended Care (K3/K4) 11:30-5:30 / $366.67 monthly
    • Extended Care (K5-12th) 3:00-5:30 / $150 monthly
    • Homework Helper - Lower School (1st-5th) / $175 monthly *10 month pay
    • Homework Helper - Upper School (6th-12th) / $175 monthly *10 month pay
