GIAA FORMS & BIG TEAMS
ACS Families,
We are introducing our new system for managing our GIAA Sports Physicals and Permission Forms for grades 6-12 here at Athens Christian. Please go ahead and create the new account and sign forms. We will upload the GIAA Sports Physical - (4 pgs) if you have given that to us.
1. Go to: https://studentcentral.bigteams.com/
2. Click Sign Up to Create New Account and complete the four step account creation
- Who is this account for? Select Parent / Guardian
- What School are you registering for? Input the name of the school that your oldest participating student attends
- Input your Personal Information for your Parent/Guardian account
- Input Username (Email) and Password
3. From the Linked Accounts page in My Profile, click "+Link Student Account"
4. Follow the 5 steps and create a student account
5. Once created, be sure to input your EMERGENCY CONTACT information (Left Navigation under My Profile), and then complete the form requirements by clicking FORMS and then ATHLETIC FORMS
6. After signing the forms as a Parent, navigate back to My Profile followed by Linked Accounts, and have your students(s) use the Sign In As button to complete any "Awaiting Athlete Signature" requirements. (forms listed below)
- Physical History Form (fill-out form and skip the upload if updated sports physical given to the Athletic Dept)
- Physical Exam Form (skip if updated GIAA sports physical given to the Athletic Dept)
- Physical Medical Eligibility Form (skip if updated sports physical given to the Athletic Dept)
- Student/Parent Sudden Cardiac Arrest Awareness Form (mandatory)
- Heat Policy Awareness Form (mandatory)
- Athens Christian Athletic Permission Form (mandatory)
In the future, you will receive reminders from Big Team Student Central for renewal of GIAA Sports Physicals & Permissions will need to be renewed annually.
